This is the pay alloment form, which was used to deduct money from a soldier's paycheck for insurance, war bonds, or to send money home to help the family. The different classes are

• Class B - For the purchase of United States War bonds.
• Class D - For the payment of premiums on United States Government Life Insurance.
• Class N - For the payment of premiums on National Service Life Insurance.
• Class E - Allotments made to individuals for the support of the alloter's families, dependent relatives, banks in the United States, or commercial life insurance companies.
• The above-described allotments are to be distinguished from class F deductions for family allowances.
